Insurance information must be provided prior to services or you will be asked to pay in full at the time of service. Our business office will be happy to help you complete your insurance forms.

Your dental plan is designed to assist you in maintaining your oral health, but rarely does it cover the full spectrum of services. Please understand that your insurance is a contract between you and the insurance company. You will be responsible for any balances not paid by your insurance company.
